First-time patients: Please complete the new patient forms and a medical history.

Returning patients: We may ask for a brief visit questionnaire and any updates to your contact or insurance information

We make every effort to see sick patients the same day they call. Routine visits should be scheduled in advance.

Dr. Kelly or an associate is available for urgent needs on evenings and weekends. In an emergency, please go to:

Hospital & Nursing Home Care

Dr. Kelly maintains staff privileges at:

  • Texas Health Harris Methodist Hospital Fort Worth
  • Baylor Scott & White All Saints Hospital

He also provides care at local nursing homes and rehabilitation facilities, including Stonegate Nursing Center and The Stayton.

Preventive & Ongoing Care

Dr. Kelly emphasizes preventive medicine and continuity of care:

  • Annual preventive exam following national guidelines (USPSTF, American Cancer Society).
  • Regular follow-ups every 6–12 months for patients with chronic conditions or on medication.
  • Same-day visits for acute illness when possible.
  • Coordination with consultants and other specialists.

Preventive care is typically covered by insurance. PMH subscribers benefit from seamless coordination and enhanced convenience.

Payment & Insurance

Our office will file insurance claims for covered services.

You are responsible for co-pays, deductibles, or non-covered services (e.g., certain vaccines, reports, or forms). PMH subscription fees are billed directly and cover added services such as convenience refills, direct messaging, and enhanced physician access.

Telemedicine

For certain conditions or family consultations, we offer secure telemedicine visits. These are billed to insurance, just like office visits.

Prescriptions & Refills

Refills are usually handled during office visits. As a PMH subscriber, you may request convenience refills on nights, weekends, or while traveling. Always bring your medication list or bottles to each appointment.
