Your Highly Capable Medical Care Specialists

At PMH, your care is directed by board-certified internal medicine specialists who are active staff members at top local hospitals.

All our clinicians are knowledgeable about the larger medical community. Years of experience complement rigorous training. Personal knowledge benefits patients from the minor (which hospital room is larger or has the best view) to the major (which cardiac surgeons have performed reliably over the years).

…thank you Dr. Kelly for your care. Your medical expertise is exceeded only by your insight and compassionate honesty. For me, you have provided the understanding and tools to confidently go on this strange, often scary, 80 plus journey…

PMH focuses on the following areas of medical care to serve you best in your needs:

Your Premium Medical Home provides preventive care and physical examination, often covered by insurance. All of our examinations are based on avoiding both under-treatment and over-treatment. We base our annual checkup on decades of experience and careful study of generally accepted guidelines for preventive care, such as those of the American Cancer Society or the U.S. Preventive Services Task Force.

And we offer more. After an exclusive executive physical, you walk away with a report, but who will be your physician if you need follow-up care or if you get sick? PMH provides you with care throughout the year. The executive physical has uncertain value, but all agree that good primary care serves the patient’s best interests. While good primary care can serve the patient’s best interests, PMH primary care elevates that care to treat you where you are physically and emotionally, to include your family circle of information when needed, and to make sure that you fully understand your care at all levels.

…just wanted to personally ‘thank you’ for your persistence in obtaining my medical record from my unexpected emergency room visit in another town. Thanks to your tenacity and care for my well-being, I feel you have protected me from some very serious consequences…

…thank you for listening to me this week. I have been so frustrated about the status and care of my mother. Your patience and guidance filled me with understanding, peace and trust…

..thank you so much for all of the time and extra calls, faxes, and emails you had to do to get my medicine!...

We are all susceptible: our good health and spirits can be troubled by illness. We need physicians to consider our symptoms, perform a physical exam, provide for objective testing like laboratory or radiography, to consider the differential diagnosis and recommend treatment or consultation.

We know that most patients prefer that their acute problem will be evaluated, diagnosed, and treated by experienced clinicians who they know and trust.

…I can’t imagine where we would have been without your care, wisdom, and comfort. You were a lifeline through some of the hardest moments of our lives. From being at our house in minutes, to visiting and bringing tea, you changed our lives. I don’t know how to thank you for more time with both of my parents and for all your reassurances along the way…

At PMH, our goal is to promptly address your medical questions. The first step can be:

  • A same-day appointment
  • A walk-in appointment
  • A phone call directly to your physician
  • Email or text your physician
  • Telehealth and virtual visits

After the physician’s evaluation, testing and treatment recommendations will be made. The clinician will describe the expected course and follow-up for your illness. It is your responsibility to follow directions and to report unexpected events, alarming symptoms, or persistence or worsening of symptoms. Results of laboratory or X-ray studies will be reported to you. If you do not receive such results, call your physician within three days to inquire. If consultation with other physicians is requested, then PMH staff will discuss consultant referral and recommendations with you.

Acute illnesses may be cared for by telehealth, an in-office visit, in the emergency room, or in the hospital. At PMH, your clinician is familiar with the hospitals, laboratories, and X-ray facilities that may be used in your evaluation. At your Premium Medical Home, your clinician can personally choose the sub-specialist physicians or surgeons needed. The choice will be based on personal knowledge of the practice and the reputation of the consultant, not based on insurance lists.

…I’m hoping you will be in charge of my care in the hospital. You know me best! It’s great to have specialists involved, but they don’t know me like you do. Thank for taking such good care of my family while I’m here including weekend calls to the kids. Not sure anyone else would have done that!...

…thank you for your wonderful care – even when I’m in the hospital. I can’t tell you how relieved I was to see you in the ER shortly after I arrived. I am very appreciative of all you do….

Geriatric patients are persons over the age of 65 who suffer chronic functional impairment caused by illness, injury, or age. The Premium Medical Home is an enhanced setting for geriatric care. The geriatric patient usually will be seen by multiple physicians and has often been hospitalized and requires home care and rehabilitation. In other offices, the care may be fragmented, with multiple clinicians providing care and no one clinician able to follow the patient across different levels of care, coordinate care, and provide information in an accessible fashion. The Premium Medical Home seeks to follow the patient across levels of care. However, such convenience, enhanced access via email and cell phone, and limited practice size is services not covered by Medicare. The PMH Subscription allows these innovative services to be made available to the patient in a reasonable fashion.

…thank you for all you did for my father and my family. From your ‘ drive-thru’ service, after hours: house calls, and everything in between, we are forever grateful for not only your compassionate care, but the respect and dignity you allowed father as his health declined. I cannot imagine a situation in which I will ever again be as emotionally challenged – nor could I ever imagine being able to endure without your support and kindness…

…We are thankful that my parents were under your care – not only for the medical care you provided, but for your wise advice and counsel to us as we struggled with life’s most difficult choices…

…upon reflection on your seeing me today, I want to express my appreciation and thanks. Particularly, I appreciate the caring, efficient, effective, and thorough way that you practice medicine. Moreover, your patience with my ignorance of things medical has helped me deal with previous doubts and misinformation…

Persons suffering chronic disease often suffer fragmented care. Care may be provided in different hospitals, or even different cities. Multiple specialists are involved in their care, and it is unusual, though welcomed, when one specialist takes particular and primary care interest in a patient. But that is the exception, not the rule.

…yesterday I spoke on the phone personally with you for the third time during these few weeks. In all of my 73 years I have never had a doctor call me three times! Thank you for the way you take time to explain things to me, for actually showing me how you make the decisions that you make, for your attention to details, and for making me feel like you truly care about me…

…thank you for all your help. We are very appreciative of how quickly you respond to our needs and requests even though we live in Texas for 6 months of the year and another state for the remaining 6 months. Wherever we are, you are responsive to our needs…

… want to express thanks for your help this past year. We have called in person, contacted you by emails, called your cell number after hours… you have consistently responded to our concerns professionally. Treating the ‘elderly’ can be exasperating, but each of you does his or her job with compassion and empathy. You have a great team in your office…

The Premium Medical Home provides patients a single physician who will coordinate care across different specialties and sites and locations of care. The PMH clinician discusses specialty care with your consultants. He or she also serves as the resource you, your family, and your other physicians will rely on for the care of serious chronic medical needs.
