
Premium Medical Home is a Subscription.
PMH Subscription enhances the patient-physician relationship beyond the office and medical facilities. Available to a limited number of patients, the PMH Subscription is a communication package that bundles a number of convenience services for one annual fee. PMH Subscription improves your connection with your clinician, adding new communication tools to keep you and your physician working together 24/7. Patients want the ability to ask questions not addressed during an office visit. We want to answer, too, but such qualitative services do not fall within the insurance model; they all take time and have value. In the past, they had to be billed directly to the patient each and every time. Insurance does not address comfort, enhanced communications, and convenience. So we designed the PMH Subscription as a simpler, easier, and better approach. It reduces barriers and encourages communication.
PMH Subscribers receive access to these services (not covered by insurance) for their annual fee:
- PMH connects you and your PMH physician through email interaction, text message communication, cell phone access, telemedicine, and in-clinic visits
- Prescription refills at your convenience—at home or on the road
- Private quarterly medicalnews written by Dr. Allan Kelly, Dr. Robert Kelly, and NP Kelly-Legler for PMH Subscribers only
- Extension of many convenience-driven communication subscription services to your out-of-town guests, without additional charge
- Simple medical forms and assistance with preauthorization
- Continuity of care across different areas, from office to home, to hospital, to consultants, your PMH physician will help you and your family
Your PMH Subscription is a streamlined, simplified system that assures communication access by a variety of means, a ‘Quality of Life’ experience that complements your ‘Quality of Care.’ It is today’s method of providing the utmost in doctor/patient relationship along with continuity of care across all medical health providers.

Ease & Convenience
The PMH subscription pays for this convenience and comfort in one annual billing. We think this is best. We want our patients to experience no barrier between them and their Premium Medical Home clinician. Comfort, convenience, and related services are not covered by insurance and are not described by insurance coding. But comfort, convenience, and added information are valuable, covered by your annual subscription that renews each year on your anniversary date.
Additionally, PMH clinicians offer a small practice size which allows the provision of working with you where you are. Clinicians see patients in the office, via telemedicine, in the hospital, other medical facilities, rehabilitation sites or your home. Your physician is available to you via phone, text, and email if you need guidance.
There is peace of mind to communicate directly and personally when you want to, from wherever you are, through a number of channels, for a single fee.
